Surgical treatment of esophageal/gastric cardiac carcinoma in patients over 75 years old

Abstract
Objective To summarize the clinical features and surgical treatment experience in patients over 75 years old with esophageal/gastric cardiac carcinoma. Methods The clinical data of 143 patients over 75 years old with esophageal/gastric cardiac carcinoma undergoing surgical treatment from Jan 2006 to Dec 2010 were retrospectively analyzed. There were 104 males and 39 females, aging from 75 to 91 years old, and 86 of the patients were complicated with various underlying diseases. All the patients underwent tumor resection via left thoracotomy, of whom 109 underwent left cervical esophagogastrostomy, 5 underwent supra-arch esophagogastrostomy, 24 underwent intra-arch esophagogastrostomy, and 5 underwent esophagojejunai anastomosis. Tubular gastroplasty was applied in the thoracic stomach construction. The patients received combined therapy in order to maintain heart and lung function and nutritional status. Results All patients went through the operation. Forty- three case- times of complications occurred, and all the patients recovered with conservative treatment except 1 case of respiratory failure. The patients were followed up for 1 to 5 years, and 6 were lost for follow - up. The 1 - year, 3 - year and 5 - year survival rate were 88. 3% , 54. 7% and 37. 8% , respectively. Conclusion Elderly patients with esophageal/ gastric cardiac carcinoma are commonly complicated with various basic diseases, which may affect the postoperative recovery. Careful surgical procedures and strengthened treatment may help most of the patients survive postoperatively.关键词
