基础胰岛素与预混胰岛素联合口服降糖药治疗2型糖尿病临床疗效比较
Comparison on the application of basal insulin and premixed insulin respectively combined with oral hypoglycemic agents in patients with type 2 diabetes mellitus

Abstract
Objective To investigate the clinical effects of basal insulin and premixed insulin respectively combined with oral hypoglycemic agents in the patients with type 2 diabetes mellitus (T2DM). Methods Forty-two patients with T2DM were randomly divided into treatment group combined with basal insulin and control group combined with premixed insulin, 21 cases in each group. Patients in both two groups received routine treatment, including oral hypoglycemic agents (OHA), regulation of blood lipid and control of diet and body weight. The treatment group combined with basal insulin was subcutaneously injected basal insulin in addition to the routine treatment, while the control group combined with premixed insulin injected premixed insulin (30R). The levels of fasting blood glucose (FBG) before and after treatment and 2-hour postprandial blood glucose (2hPG) were detected, and the level of glycosylated hemoglobin (HbAlc) was determined by microcolulmn method. The doses of insulin injection were compared in the two groups, and the recovery of blood glucose and occurrence of hypoglycemia were observed. Results Compared with before treatment, the levels of FBG, 2hPG and HbAlc all conspicuously decreased in both groups, and the decreased range of treatment group combined with basal insulin was larger. The insulin dose of treatment group combined with basal insulin was significantly less than that of the control group combined with premixed insulin. During the treatment, patients in both groups suffered from mild hypoglycemia and alleviated after taking food. Conclusion In the aspects of controlling FPG and HbAlc, the therapeutic effects of basal insulin combined with OHA are better, and insulin dose is less.关键词
