创伤性浮膝的手术治疗
Surgical Treatment of Traumatic Floating Knee
摘要
Abstract
Objective To explore the clinical features and the surgical treatment of traumatic floating knee. Methods The clinical data of 30 cases of floating knee underwent surgical treatment were analyzed retrospectively. According to fracture site, type and soft tissue injury, the femoral fractures were fixated by pressure plate in 8 cases, anatomical plate in 4 cases, locking plate in 6 cases and interlocking nail in 12 cases ; the tibial fractures were fixated by pressure plate in 5 cases, anatomical plate in 6 cases, locking plate in 4 cases, interlocking nail in 10 cases and exteral fixator in 5 cases. Results 30 cases were followed up for 2 to 5 years with an average of 32 months. 26 cases were obtained fracture healing in 5 to 12 months (mean 7 months) after operation. 2 cases were found tibial non-union and achieved healing after reoperation with bone graft and internal fixator. One case was affected by tibial infection and healed after debridement, drainage and change dressings. Another one case was found tibial plate exposure combined anterior tibial muscles necrosis generally, and gained bone union by debridement and change dressings for half year and soft tissue healing after remove the plate, but remained drop foot because of anterior tibial sticking bone cicatrix. According to Karl-strom evaluation criterion, the knee joint function revealed excellent in 16, good in 10, fair in 2 and poor in 2. The rate of excellent and good was 86.7%. Conclusions The operative methods of floating knee should be selected according to fracture site, type and soft tissue injury. Postoperative functional exercise would be performed as early as possible to prevent complications.关键词
