摘要

Abstract

Objective To provide anatomical evidence for surgery by observation of the pterygopalatine fossa( PPF ) under endoscopy. Methods 8( 16 sides ) adult cadaveric heads were dissected via endoscopic middle meatal trans-palatine approach, and maxillar sinus rear wall approach. Comparing two different surgical approach, we observed operations and related structures in the path's proximity to major blood vessels and nerves. Results The lateral PPF can be well exposed via maxillary sinus rear wall, the medial PPF can be well visualized via endoscopic middle meatal transpalatine approach; Internal maxillary artery( IMA ) branch of PPF varied largerly. There were five branches after the posterior wall of maxillary sinus, the first branch being the anterior deep temporal artery. IMA branches separated in order. Infraorbital artery( IOA ) and posterior superior alveolar artery( PSAA ) had a common stake with about 50 percent ( 8/16 ), separated stake from IMA was about 50 percent( 8/16 ). IMA, descending palatine artery( DPA ),and sphenopalatine( SPA ) shaped like "Y" was about 37. 5 percent( 6/16 ),"M"accunts for 12. 5 percent( 2/16 ),and "intermediate "50 percent( 8/16 ). Foramenrotundum( FR ), pterygoid canal( PC ) and spheno-palatine foramen( SPF ) were important bony landmarks of PPF. The PC points to internal carotid artery( ICA ). Conclusion The two surgical approaches can discover different pterygopalatine fossa. It is essential to be acguain-fed with pterygopalatine fossa and the surrounding anatomic landmarks and IMF branches of running discipline, and appropriate chose of corresponding surgical approach processing different range of PPF lesions will be helpful for exposure and effective resection of lesions to avoid unnecessary surgical trauma and complication.
