脓毒症急性肾损伤患儿T细胞亚群变化及临床意义
Changes of T-cell Subsets and Its Clinical Significance in Children with Septic Acute Kidney Injury

Abstract
Objective:To observe the changes of T-cell subsets in children with sepsis and septic acute kidney injury,and to discuss its clinical significance.Methods:Forty-two cases of children with septic acute kidney injury and fifty cases of children with sepsis were selected.At the same period thirty healthy children were enrolled as the control group.T cell subsets (CD3 + T,CD4+ T and CD8+ T and CD4+ T/CD8+ T in peripheral blood were measured by flow cytometry.The pediatric critical illness score (PCIS) were graded within twenty-four hours after admission.Results:In the septic acute kidney injury group,CD3+ T (43.56％±8.87％),CD4+ T (25.01％ ±6.18％) and CD8+ T (16.52％ ±4.00％) cells were significantly lower than those in the control group and the sepsis group (P＜0.05) ; CD4+ T/CD8+ T (1.71％±0.42％) were significantly higher than those in the control group and the sepsis group (P＜0.05).In the septic acute kidney injury group,CD3 + T (36.51％ ±8.47％),CD4+ T (17.36％ ±3.28％) and CD8 + T (14.12％ ±2.23％)cell and PCIS (58.67±2.58) in the patients with stage Ⅲ were lower than those with stage Ⅰ and stage Ⅱ ; the difference wasstatistically significant between patients with stage Ⅲ and stage Ⅰ (P＜0.05).In the septic acute kidney injury group,CD3+ T(35.67％ ±9.23％),CD4+ T (16.56％ ±5.27％),CD8+ T (13.85％ ±4.34％),CD4+ T/CD8+ T (1.09％ ±0.48％) and the PCIS (55.64±3.56) in dead patients were lower than those in survival patients; the differences were statistically significant (P＜0.05).Conclusions:Children with sepsis and septic acute kidney injury have varying degrees of suppression of cellular immune function.The degree of immunosuppression and severity and prognosis of the disease have a significant correlation.Detection of T lymphocyte subsets has important clinical significance in the assessment of immune function of patients with sepsis and judging the severity of the disease.关键词
