中低温低流量体外循环在婴儿心内直视手术的临床研究

Abstract
Objective:To investigate safety and feasibility of the application of mild & moderate hypothermic and low-flow extracorporeal circulation while undergoing open-heart surgery to the infants with congenital heart disease.Method:40 sick babies admitted to our department who undergoing open-heart surgery were randomly divided them into obsewation group and control group.The 20 sick babies in control group were applied mild hypothermia and high-flow perfusion technique for open-heart surgery,while the other 20 sick babies in observation group with moderate hypothermia and low-flow perfusion technique for open-heart surgery.Compared the postoperative brain function and the intraoperative blood gas of the two groups.Result:The moderate hypothermia and low-flow perfusion venous oxygen saturation was above 80% without hypoxia acidosis blood gas,and blood lactate didn't elevate.There is no significant difference (P> 0.05) on the lactate concentration of the artery blood and internal jugular vein blood of the observation group and the control group at each time.The postoperative recovery was good without significant cardiopubnonary bypass-related complications.Conclusion:The application of mild & moderate hypothermic and low-flow extracorporeal circulation while undergoing open-heart surgery to the infants not only can satisfy the need of oxygen supply of the brain tissue,but also reduce the destruction to the blood,keep the surgical area clear and help to reduce the adverse reactions led by long time high-flow perfusion.It provides new means to carry out clinical brain protection of infants with complex congenital heart surgery.关键词
