特发性肺纤维化患者BALF中PPAR-γ、NF-κB表达及细胞学分类与肺动脉高压相关性研究
Correlation between cell sorting,PPAR-γ, NF-κB expression in bronchoalveolar lavage fluid and pulmonary hypertension in idiopathic pulmonary fibrosis patients

Abstract
Objective To investigate the correlation between cell sorting,PPAR-γ,NF-κB expression in bronchoalveolar lavage fluid (BALF) and pulmonary hypertension in idiopathic pulmonary fibrosis(IPF)patients.Methods Doppler ultrasound echocardiography was applied to measure the pulmonary artery systolic pressure in 45 IPF patients.All patients were divided into pulmonary hypertension group and non-pulmonary hypertension group according to the level of pulmonary artery systolic pressure.Bronchoalveolar lavage was examined in all patients.HE staining was used to analyze the cell sorting in recovered BALF.Expression of PPAR-γ and NF-κB in BALF was determined by enzyme linked immunosorbent assay.Cell sorting and PPAR-γ,NF-κB expression in BALF were compared.Correlation between cell sorting,PPAR-γ,NF-κB expression in BALF and pulmonary artery systolic pressure was analyzed by Pearson correlation analysis.Results Of 45 patients,15 (33.3%) were found pulmonary artery systolic pressure ≥40 mmHg.Percentage of neutrophils and NF-κB expression levels in BALF in pulmonary hypertension group were significantly higher than those in non-pulmonary hypertension group(P < 0.05).PPAR-γ expression in BALF in pulmonary hypertension group was significantly lower than that in non-pulmonary hypertension group(P < 0.05).Percentage of neutrophils and NF-κB expression level in BALF of IPF patients were positively correlated with pulmonary artery systolic pressure(r =0.411,P =0.022 ; r =0.363,P =0.045).PPAR-γexpression in BALF was negatively correlated with pulmonary artery systolic pressure (r =-0.635,P =0.000).Conclusion Increased percentage of neutrophils,reduced expression of PPAR-γ and enhanced expression of NF-κB in BALF of IPF patients play a certain role in the pathogenesis of IPF with pulmonary hypertension.关键词
