Abstract
Objective To study the feasibility of using N-terminal pro-brain natriuretic peptide (NT-proBNP) as early predictive factor for prognosis of sepsis.Methods One hundred patients with sepsis admitted from Jan,2008 to Dec.2012 were retrospectively analyzed.Age,gender,past medical history,NT-proBNP,cardiac troponin Ⅰ (cTnⅠ),arterial blood lactic acid,C-reactive protein (CRP),procalcitonin (PCT),time from onset to death in patient died were recorded.Receiver operating characteristic (ROC) curve was plotted in accordance with the result of COX regression analysis,and the correlation between NT-proBNP and prognosis of sepsis was analyzed.Results COX regression analysis showed that age [relative risk(RR)=1.027,95%confidence interval(CI):1.002-1.052,P=0.033],NT-proBNP (RR=1.606,95% CI:1.190-2.267,P=0.002),lactic acid (RR=1.718,95% CI:1.250-2.361,P=0.001) were the correlation factor of 28-day death.The ROC curves showed that age [area under curve (AUC) 0.656,P<0.01],NT-proBNP (AUC 0.772,P<0.001),lactic acid (AUC=0.775,P<0.001) were positively correlated with 28-day death.The fit-curve of NT-proBNP and lactic acid showed linear correlation (r2=0.663,P<0.001); the correlation between NT-proBNP and time from onset to death was not linear.Conclusions NT-proBNP was correlated with the prognosis of sepsis,but was not linearly correlated with time from onset to death.The early rise of NT-proBNP might be helpful for indicating the critical condition of sepsis patient.关键词
