腹腔镜手术治疗妊娠期卵巢良性肿瘤有效性和安全性分析

Abstract
Objective To disscuss the efficiency and safety of laparoscopic management on patients of benign ovarian tumors during pregnancy.And to analyze the outcomes of pregnancy and descendants after a long-term follow-up.Methods The data of 17 patients hospitalized in the First People's Hospital of Foshan who underwent laparoscopic surgery for benign ovarian tumors during pregnancy between January 2007 and December 2010 were retrospectively analized.Patients’ general condition,perioperative characteristics,outcome of pregnancy and descendants were assessed.Results Of the 17 women included,mean maternal age is (24.3 ±3.9) years old(19 ～34y),mean gestational age is (12.9± 3.3) weeks (6 ～ 18W) and mean duration of operation is (114.0 ± 42.3)min(60 ～210 min).Postoperative pathologic outcomes indicated mature teratoma for 9 cases in which 3 were bilateral,mucinous cystadenoma for 4 cases,and follicular cyst,parovarian cyst,endometriosis cyst each for 1 case.One patient without pathologic result underwent operation for torsion reset ofcorpus luteum cyst.No discomfort,complication or miscarriage was found postoperatively.The mean gestational age at delivery is 38.2 ± 1.1 weeks and mean birth weight is(3050 ± 180) g(2800 ～ 3500 g).After a follow-up of the descendants until yound children (within 3 years old) since delivery,no growth abnormalities or dysplasia was detected by the use of Gesell Test.No statistical difference between the surgery group and control group was found.Conclusion Laparoscopic surgery for benign ovarian tumors during pregnancy is safe and effective,which does not affect the outcome of pregnancy and descendants.关键词
