腰丛-坐骨神经联合阻滞在老年危重患者下肢手术中的应用价值
Application of Combined Lumb Plex and Sciatic Nerve Block in the High Risk Elderly Patients with Lower Extreme Surgery

Abstract
Objective:To explore the applied value of combined lumb plex and sciatic nerve block by the guide of nerve stimulator in the high risk elderly patients with lower extreme surgery.Method:57 cases the aged patients with more complications who were plan to receive lower extreme surgery were ramdonly divided into combined lumb and sciatic group(groupA),CSEA group( groupB)and general anesthesia group(groupC)according to the anesthesia methods on them .Recorded and compared the varied of patients on HR,SBP,DBP,SpO2 and adverse reactions before anesthesia, 15,30,60 minutes after anesthesia .Result:GroupB、C patients had lower blood pressure within 30 minutes after anesthesia than group A and needed to rise blood pressure treatment.There were 3 cases having nausea and vomiting ,10 cases urinary retention ,5 cases low back pain in group B. There were 4 cases having cognitive disorder after operation in group C.But there was not the phenomenons above in group A .Conclusion:It can provide higher success rate,steady hemodynamic,less complication ,longer analgesia that the combined lumb plex and sciatic nerve block by the guide of nerve stimulator and is a better anesthesia method for high risk elderly patients with lower extreme surgery.关键词
