妇科腹腔镜手术并发症43例临床诊治分析

Abstract
Objective To investigate the causes of complications of gynecological laparoscopic surgery , treatment strategies and preventive measures.Methods A retrospective analysis was done on 2 985 cases of gynecological laparoscopic surgery in Jiangyin People ’ s Hospital between January 2009 and January 2013.Results Totally 43 of them developed complications , with the incidence of 1.44%.There were 10 cases converting to laparotomy , and the incidence rate was 0.34%.In different types of surgery , the incidence rate of complications of hysterectomy (4.89%) was highest compared with that of accessories operation (0.42%), and the difference was statistically significant (χ2 =56.75, P<0.05).But there was no significant difference in the incidence rate of complications between uterine fibroids stripping operation (0.34%), hysteroscopy combined with laparoscopic surgery (2.00%) and accessories operation (0.42%) (χ2 value was 0.01 and 2.61, respectively, both P>0.05).The incidence rate of other surgery complications was higher than that of accessories operation , and the difference was statistically significant (χ2 =5.10, P<0.05).All of the complications were diagnosed timely and treated effectively with good prognosis .Conclusion Laparoscopic surgery is increasing year by year , so are surgical complications .Surgical complications are related with the difficulty of surgery and operators ’ skills.So evaluation should be made before surgery , indications of surgery should be mastered strictly , operating techniques must be grasped skillfully and convert to laparotomy timely in surgery , so that the complications of surgery can be avoided as far as possible .关键词
