Abstract
Objective To investigate the influence of the blood glucose fluctuation on the incidence of dia-betic nephropathy in patients with type 2 diabetes.Methods Continuous glucose monitoring system (CGMS)for 72 h was applied for 136 patients with type 2 diabetes mellitus.All diabetes mellitus were divided into non diabetic ne-phropathy group (NDN group)and patients with early DN (DN group)according to the ratio of urinary albumin to creatinine.The levels of 72 h average blood glucose,blood glucose fluctuation and HbA1 c,blood pressure,blood lipid were recorded and analysed.Results (1)There was significant difference of blood pressure,fasting blood glucose, the 24 h average blood glucose level(24hMBG),average blood glucose excursion amplitude(MAGE)and HbA1 c be-tween the two groups (P <0.05).(2)The results of Logistic analysis showed that 24hMBG and MAGE were inde-pendent risk factors to the incidence of early diabetic nephropathy,with the OR of 0.563 and 1.220(P =0.005and 0.011),respectively.Conclusion Glucose fluctuation is a important risk for the development of diabetic nephropathy.关键词
