急性闭角型青光眼合并白内障的不同术式治疗疗效对比研究

Abstract
Objective:To use different operative methods for treatment of acute angle-closure glaucoma with cataract and compare the curative effect.Method:Sixty-eight patients(71 eyes)with glaucoma with cataract were randomly divided into the group A for 34 cases(35 eyes)and the group B for 34 cases(36 eyes)by double-blind method according to the order of the treatment.The group A was treated with simple trabeculectomy,the group B was treated with glaucoma-cataract joint operation,the intraocular pressure and visual acuity of pre-operation and post-operation were observed and compared,and paid attention to observing complication and curative effect in the two groups at the time of follow-up.Result:Effective rate of the Group A was 97.14%,effective rate of the group B was 97.22%,the differences in excellent rate between the two groups had no statistical significance(Ridit z=4.264,P=0.089);Compared with preoperative,the intraocular pressure,vision of two groups of patients were improved after surgery(P<0.05).And intraocular pressure of the group A(15.74±6.85)mm Hg was higher than group B(12.65±4.74)mm Hg,the difference was statistically significant (P<0.05);Vision of the Group A(0.29±0.18)D was lower than group B(0.71±0.35)D,the difference was statistically significant(P<0.05);5 eyes in group A with complications(14.28%),6 eyes in group B with complications(16.67%),there was no statistically significant difference in the two groups(Fisher P=0.091).Conclusion:For patients with acute angle-closure glaucoma with cataract,can choose the joint surgery,in order to improve the patients postoperative visual acuity,ensure the operation effect and clinical effect.关键词
