CDE、CDUS检查评估活动期强直性脊柱炎患者骶髂关节炎的价值

Abstract
Objective To evaluate the application of color Doppler ultrasonography(CDUS) and color Doppler energy im-age(CDE) in diagnosis of sacroili tis of patients with active ankylosing spondylitis (AS). Methods Sixty three sacroiliac joints with active inflammatory in 36 patients with active AS (active AS group) and 50 sacroiliac joints in 25 healthy subjects (control group) were examined by CDUS and CDE. Degree of blood flow within and around the sacroiliac joints were evaluated by semi- quanti-tative scale (0- 4 grade), and the resistance index (RI) was also measured.The blood flow demonstration rate,degree of blood flow and blood flow RI were compared between two groups.Fifty four sacroiliac joints in 31 patients with inactive AS after treat-ment (inactive AS group) were also examined by CDUS and CDE. The same indexes were compared between active AS and in-active AS groups. Results Higher blood flow demonstration rate was found in patients with active AS(57/63, 90.5%) than in con-trol group (26/50, 52.0%)(P=0.000).The degree of blood flow within and around the sacroiliac joints between these two groups were statistical y different (P=0.000), there were mainly grade 2- 3 in AS patients and grade 0- 1 in contro1s. Mean value of RI in active AS patients was 0.57±0.07, which was lower than that in controls (0.78±0.11, P=0.000) and in inactive AS patients(0.69± 0.09, P=0.000).There were no significant differences in blood flow demonstration rate and degree of blood flow between active and inactive AS groups. Conclusion The abnormal flow signs at the sacroiliac joints in AS patients can be detected by color ul-trasonography.Low RI value provides evidence for differential diagnosis between active and inactive AS sacroili tis.关键词
