摘要
Abstract
Objective:To know the effect of self-management model and the case management model on protection and control of diabetes mellitus(DM) in community.Method:200 cases of type 2 DM patients in community were randomly selected to receive self-management model and the case management model. Before and after 1 year intervention,the changes of relative indexes were compared.Result:After intervention,the knowledge of DM patients was significantly increased(P<0.01).The indexes of body mass index,systolic blood pressure,diastolic blood pressure,triglyceride were obviously improved(P<0.01).Conclusion:The combination of self-management model and the case management model is helpful to upgrade the quality of community health management,it ultimately achieves an ideal control effect.关键词
