带皮神经的掌背动脉穿支皮瓣修复手指软组织缺损的临床研究
Clinical study of dorsal metacarpal artery perforator flap with cutaneous nerve in the repair of finger soft tissue defect

Abstract
Objective To explore the clinical effect of dorsal metacarpal artery perforator flap with cutaneous nerve in the repair of finger soft tissue defect.Methods 60 patients with single finger soft tissue defect in our hospital from January 2012 to September 2014 were selected as the research objects,and randomly divided into dorsal metacarpal artery perforator flap group (group A,n=42) and conventional repair group (group B,n=18) by simple sampling method.Group B was treated with conventional abdominal skin flap repair,while group A was treated with dorsal metacarpal artery perforator flap with cutaneous nerve repair.The primary healing rate,survival rate,healing time,postoperative complications of two groups were compared,and hand function assessment (TAM score) and life quality (SF-36 score) were recorded.Results ① After treatment,the survival rates of flap in both two groups were ideal (P ＞0.05); the primary healing rate of group A was 88.1％,and healing time was (0.9 ± 0.1) years,significantly better than 61.1％ and (1.2 ± 0.2) years of group B (P ＜ 0.05).② TAM score of group A was (14.4 ±1.1),significantly higher than (10.3 ± 1.2) of group B (P ＜ 0.05).After treatment,there were statistically significant differences in social function and mental health scores between two groups (P ＜ 0.05),no statistically significant difference in body pain score.③ The rate of postoperative complications in group A was 2.38％,significantly lower than 16.67％ in group B (P ＜ 0.05).Conclusion Dorsal metacarpal artery perforator flap with cutaneous nerve in the repair for patients with finger soft tissue defect can effectively restore hand function with a high success rate,worthy of clinical promotion.关键词
