Abstract

Objective To investigate the safety and accuracy of minimal invasive screw fixation for Anderson and D′Alonzof Ⅱ odontoid fractures under the real -time three -dimensional navigation guidance .Methods 36 patients were treated with anterior odontoid screw fixation for odontoid fracture , cannulated screw fixation with minimal invasive under real -time Orbic -3D navigation was performed on 21 cases, while open cannulated screw fixation with traditional C -arm fluoroscopy was performed on 15 cases.The JOA score and VAS before and after surgery , operative time, fluoroscopy time, blood loss, postoperative X -ray, screw position in CT classification and fracture healing at final follow -up were recorded.Surgical features, clinical efficacy and safety, and accuracy of pedicle screws were compared .Results Signifi-cant shortened operative time and fluoroscopy time , less blood loss, and lower screw penetration rate in CT scan were ob -served in navigation group (P <0.05).There was no significant difference in JOA or VAS between the two groups before and after surgery (P >0.05).Conclusion The real -time Orbic -3D navigation system guided minimally invasive in -ternal fixation of hollow nails for Anderson and D ′Alonzof type Ⅱ odontoid fracture is simple, minimal invasive, and can shorten the operation time, intraoperative fluoroscopy time and ensure condylar screw placement accuracy and safety .
