Abstract
Objective To investigate the relationship between the abnormal circadian rhythm of blood pressure and the renal damage in patients with primary hypertension .Methods A total of 154 patients with primary hypertension who were treated and underwent 24-h ambulatory blood pressure monitoring in our hospital from June 2012 to February 2014 were enrolled in this study .They were divided into four groups with 51 in dipper group , 64 in non-dipper group , 32 in anti-dipper group and 7 in deep-dipper group.The abnormal circadian rhythm of blood pressure was observed and compared among the groups.The urinary micro albumin (mALB), blood urea nitrogen (BUN), se-rum creatinine (SCr) and uric acid (UA) were detected in all patients.Results There was no sig-nificant difference in the daytime blood pressure among the four groups ( P>0.05) .The nocturnal systolic and diastolic blood pressure was significantly higher in non-dipper group , anti-dipper group and deep-dipper group than in dipper group ( P<0.05) .The 24-h pulse pressure was also signifi-cantly increased in anti-dipper group when compared with deep-dipper group ( P<0.05) .Signifi-cant difference was noted in the mALB , BUN and SCr except UA among the four groups (P<0.05) .Conclusion The abnormal circadian rhythm of blood pressure is correlated with the renal damage in patients with primary hypertension .Patients with a non-dipper, anti-dipper or deep-dipper pres-sure profile are more likely to develop early renal damage .关键词
