Abstract
Objective To explore the effects of intensification therapy with insulin pump on blood glucose fluctu-ations, oxidative stress injury and ulcer healing in elderly patients with type 2 diabetes mellitus (T2DM) and foot ulcer. Methods A total of 80 T2DM elderly patients with foot ulcer admitted during June 2012 and June 2014 were randomly divided into observation group (n = 40) and control group (n = 40). The control group received conventional treatment with subcutaneous insulin injection, while the observation group received the short-term intensification therapy with insu-lin pump, and the two groups were treated for 2 weeks. The standard deviation of blood glucose (SDBG), the mean am-plitude of glycemic excursions (MAGE), the largest amplitude of glycemic excursions (LAGE), plasma malondialde-hyde (MDA) content, the superoxide dismutase (SOD) level, 8- Prostaglandin F2alpha (8-iso-PGF2α) level and ulcer area before and after treatment in the two groups were recorded. Results Compared with those before treatment, the lev-els of SDBG, MAGE and LAGE were significantly decreased after treatment in the two groups ( P < 0. 05), and de-creased MAGE level in the observation group was more obvious than that in the control group (P < 0. 05); the indexes of oxidative stress and ulcer area in the 1st and 2nd week of treatment in the two groups were significantly improved compared with those before treatment (P < 0. 05), and the improvement in observation group was more obvious than that in control group (P < 0. 05); after treatment for 2 weeks, the healing rate of ulcer in observation group was significantly higher than that in control group (P < 0. 05). Before and after the treatment, MAGE and ulcer area were positively correlated with MDA and 8-iso-PGF2α (P < 0. 05), while MAGE and ulcer area were negatively correlated with SOD (P < 0. 05), and the ulcer area was positively correlated with MAGE before and after the treatment (P < 0. 05). Conclusion Intensifica-tion therapy with insulin pump in treatment of elderly T2DM patients with foot ulcer can well control blood glucose fluctua-tion, reduce oxidative stress level and accelerate ulcer healing.关键词
