胰岛素泵治疗溃疡型糖尿病足感染患者的临床疗效分析
Clinical Effect of Insulin Pump on Patients with Infected Diabetic Foot Ulcers

Abstract
Objective:To investigate clinical curative effect of insulin pump on patients with infected di-abetic foot ulcers , and provide reference for clinical prevention and treatment .Method:84 cases with infec-ted diabetic foot ulcers in our hospital from February 2011 to February 2014 were randomly divided into con-trol group (n=42 cases) and treatment group (n=42 cases);both groups were received anti-infective ther-apy, the control group was given multiple subcutaneous insulin injection ( MSⅡ) treatment, the treatment group was given continuous subcutaneous insulin infusion ( CSⅡ); ulcer area , fasting plasma glucose ( FPG) , C-reactive protein ( CRP ) and white blood cell count ( WBC count ) were recorded before treat-ment and after 1 week, and after 2, 4 week’ s treatment;Ulcer healing were recorded after week ’ s treatment and related factors for ulcer healing were analyzed .Result:Compared with control group , ulcer area of treat-ment group significantly reduced after 2, 4 week’s treatment (P <0.05); FPG, CRP and WBC of treat-ment group decreased obviously after 1, 2, 4 week’s treatment (P <0.05);After 4 week’s treatment, good rate of ulcer healing in treatment ( 88.1%) was significantly higher than control group ( 66.7%) (χ2=5. 509,P=0.019);The multi-factor Logistic regression analysis showed that the baseline ( before treatment ) FPG, CRP and WBC and FPG , CRP and WBC after 4 week’ s treatment were positively correlated with ul-cer healing (P <0.05).Conclusion: Insulin pump can effectively control blood glucose levels in patients with infected diabetic foot ulcers , suppress inflammation , prevent and control spread of infection so as to pro-mote ulcer healing .关键词
