Abstract
Objective To explore the application effect of continuity of care to the hospital and patients with chronic obstructive pulmonary disease. Methods 86 patients with stable chronic obstructive pulmonary disease patients were randomly divided into observation group and control group,43 cases in each group.The control group were received rou-tine discharge guidance,the observation group were received continuing nursing care. Results After 1 year,the acute at-tack rate of the observation group was 4.65%,was lower than 25.58% of the control group,the difference was significant (P<0.05).The score of life quality of the observation group was (4.1±1.2),was lower than (18.6±7.4) of the control group, the difference was significant (P<0.05). Conclusion Take the continuity of care can significantly reduce the acute exac-erbation rate in patients with stable chronic obstructive pulmonary disease,improve the quality of life.关键词
