Abstract
Objective To compare the application effect of suture anchor fixation and titanium miniplate fixation in cervical posterior unilateral open-door expansive laminoplasty. Methods From January 2010 to June 2012,cases treated by cervical posterior unilateral open-door suture anchor fixation and cervical posterior unilateral titanium miniplate fix-ation in multi-section cervical spondylotic myelopathy were retrospectively analyzed.The differences in operation time, intraoperative amount of bleeding,postoperative JOA score,narrowest spinal canal area,and complications between two groups was compared respectively. Results There were statistical differences in operation time and intraoperative amount of bleeding between two groups (P<0.05).With regard to JOA score,there was no statistical difference between two groups before and one week after surgeries (P>0.05).Two years after surgery,there was statistical difference between two groups (P<0.05).The score in titanium miniplate group was higher than that in suture anchor fixation group.There was no statistical difference in narrowest spinal canal area before and one week after surgeries between two groups (P>0.05).Two years after surgery,there was statistical differences between two groups (P<0.05).The narrowest spinal canal area in titanium miniplate fixation group was wider than that in suture anchor fixation group.Incisions were healed in first grade in the two groups,and there were no such severe complications like wound infection or leakage of cere-brospinal fluid. Conclusion Two fixations of cervical posterior unilateral open-door expansive laminoplasty both can in-crease the spinal canal area of lesions in patients with spinal cord pressed and effectively improve neurological func-tion.There are some shortcomings such as long operation time and large amount of bleeding in titanium miniplate fixa-tion,but it can effectively prevent the door reclosed and obtain a better long-term effect.关键词
