颈后路内固定治疗不稳定型Hangman骨折

Abstract
Objective To explore clinical efficacy of posterior pedicle and lateral mass screw system in treatment of unstable Hangman fracture.Methods A retrospective review of 17 cases of Hangman fracture was performed.By Levine-Edwards classification,there were 7 cases of type Ⅱ,8 cases of type Ⅱa,2 cases of type Ⅲ.Routine preop-erative skull traction reduction of the fracture after admission was used,and followed by posterior C2 pedicle screw with C3 lateral mass screw fixation.Neck collar protection was advised for 6 weeks in postoperation.Results No spi-nal cord injury,nerve injury,vertebral artery injury were found after surgery.17 cases were followed up for 12 ~44 months,the fracture healing time was 3 ~6 months.Neck pain,numbness and other symptoms disappeared.Neck mobility slightly decreased than normal.No fixation failure and infection complications were found.Conclusions Posterior pedicle and lateral mass screw system in treatment of unstable Hangman fracture can obtain satisfactory clini-cal efficacy.关键词
