全髋关节置换合并转子下短缩截骨治疗 Crowe Ⅳ型成人髋关节发育不良的近期疗效

Abstract
Objective To study the clinical effect of total hip arthroplasty (THA) with subtrochanteric shortening os-teotomy for treatment of Crowe Ⅳ developmental dysplasia of hip (DDH) in adults. Methods 20 patients of CroweⅣ DDH (24 hips) were studied. The unified operation mode was THA with subtrochanteric transverse shortening os-teotomy. Change of leg length and Harris score before and after operation, replacement complications and the iconog-raphy of hip after operation were recorded and analysed. Results 20 patients were followed up for 12 ~ 30 months. All operation incisions were healed in Ⅰ stage without infection. No patients experienced clinical manifestation of deep vein thrombosis or sciatic nerve injury. 1 patient experienced late healed of osteotomy after operation. In the third months, the patient got bone healing after autologous bone graft in Ⅱ stage. No obvious prosthesis loosening or dislocation at last follow-up. Before operation, 20 patients had different degrees of claudication, at last follow-up, on-ly 2 patients had mild claudication, the difference was statistically significant(P < 0. 05). Preoperative bilateral limb length discrepancy was 2. 0 ~ 6. 5 (4. 6 ± 1. 7) cm, at last follow-up, it was 0. 5 ~ 1. 0(0. 7 ± 0. 2) cm, the differ-ence was statistically significant(P < 0. 05). Preoperative Harris score was 36 ~ 68 (46 ± 8) points, at last follow-up, Harris score was 78 ~ 96 (90 ± 3) points, the difference was statistically significant(P < 0. 05). Conclusions THA with subtrochanteric shortening osteotomy treatment of Crowe Ⅳ DDH contributes to the restoration of normal hip center, reduces the difficulty of prosthesis replacement and the possibility of complications such as vascular or nerve injury after operation. Its recent curative effect is reliable and safe.关键词
