Abstract
Objective To compare the efficacy and safety of mirtazapine and escitalopram oxalate in the treatment of elderly de-pression with insomnia. Methods 68 elderly depression patients with insomnia were divided into the research group( n=35,treated with mirtazapine)and the control group(n=33,treated with escitalopram oxalate). All groups were treated for 8 weeks. At the begin-ning of the treatment,mirtazapine dose were 7. 5~15 mg/d,and escitalopram oxalate group dose were 5 mg/d. After 8 weeks treat-ment,mirtazapine maximum 45mg/d,and escitalopram oxalate dose 20 mg/d. The clinical efficacy were assessed with the Hamilton Rating Scale for Depression(HRSD)and the sleep quality with Pittsburgh Sleep Quality Index(PSQI)and adverse effects with the Treatment Emergent Symptom Scale(TESS)before treatment and at the ends of the 1st,2nd,4th,8th week treatment. Results At the end of the 8th week,effective rates were respectively 91. 4% in the research group and 90. 9% in the control group,which showed no significant differences(χ2 =0. 124,P﹥0. 05). At the end of the 8th week,the PSQI scores of mirtazapine was(6. 20 ± 2. 52)and es-citalopram oxalate was(9.14 ±2.42),two groups showed statistically difference(t=4.98,P﹤0.01). The side effect scores of the two groups showed no significant difference(χ2 =1. 32,P﹥0. 05). Conclusion Compared with escitalopram oxalate,mirtazaine was e-qually safe and effective in the treatment of elderly depression with insomnia,but mirtazaine showed a significant advantage in the im-provement of insomnia.关键词
