Abstract
Objective To investigate the clinical outcome of microsurgical treatment of giant parasagittal and falcine men-ingiomas located in the middle third of the superior sagittal sinus, and to further discuss the treatment strategy of this disease. Methods From Jan. 2004 to May. 2013, 23 patients suffering from giant parasagittal and falcine meningiomas located in the middle third of the superior sagittal sinus were treated in our hospital. The clinical data, the microsurgical procedure and the treat-ment strategies were retrospectively analyzed. Results In the present series, lateral wall and roof of the sinus was involved by tumors in 15 patients, and wall of sinus was invaded in 4 patients (entire sinus occluded in 1 patient); tumors arising from the falx without sinus involvement were observed in 4 patients. Simpson grade I, II and III resection were achieved in 5, 15 and 3 pa-tients, respectively. Preoperatively, 9 patients were suffering from partial hemiparesis, in which 7 patients were improved postopera-tively, while the other 2 patients suffered from a short-term deterioration and returned to preoperative status after 3 months. During 21. 3 months(range from 3 to 72 months) of follow-up, the MRI showed the residual tumors were detected in 3 patients and no tumor recurrence were observed. Conclusion The key to microsurgical treatment of giant parasagittal and falcine meningiomas located in the middle third of the superior sagittal sinus is protection of functional cortex and critical venous structures. Skillful microsurgical techniques and rational treatment strategy can achieve favorable outcomes and improve the surgical safety.关键词
