经皮经肝胆囊穿刺引流后择期行腹腔镜胆囊切除治疗老年急性重症胆囊炎的临床观察

摘要
Abstract
Objective To investigate the clinical effect of percutaneous transhepatic gallbladder drainage combined with laparoscopiccholecystectomy in severe acute cholecystitis for the elderly patients. Methods The total 100 elderly acute cholecysti-tis patients wene diagnosed with Tokyo guidelines. Patients were divided into group A( grade Ⅰand Ⅱ) according to the patients’ severity and group B( grade Ⅲ) . The patients in group A were treated with laparoscopiccholecystectomy and those in group B were treated with percutaneous transhepatic gallbladder drainage and then laparoscopiccholecystectomy. The clinical effect were observed and recorded. Results Laparoscopiccholecystectomy were performed in all patients. There were no significant difference in 2 groups in complications,surgery time and the open surgery rate(P>0. 05). Conclusion The percutaneous transhepatic gallblad-der drainage combined with laparoscopiccholecystectomywere effective and safe in elderly patients with severe acute cholecystitis and worthy to wide spread in clinincal use.关键词
