Abstract
Objective:To compare the two different approaches ( paraspinal muscle approach and conventional posterior median approach) in the treatment of no neurological symptoms ( Frankel grade E) the clinical curative effect of thoracolumbar burst frac-ture.Methods:A retrospective analysis is used in the 40 cases who suffering during pedicle screw fixation during 2010 March to 2013 July in our hospital.All cases are Frankel grade E thoracolumbar burst fracture patients, there were 20 cases used the conven-tional posterior median approach, the other 20 cases used the paraspinal muscle approach, Compared the operation time, the amount of bleeding, postoperative drainage, postoperative bed time, preoperative waist backache ( Visual Analogue Scale, VAS) score and postoperative 72 H VAS scores, the improvement of the convex Cobb angle and the anterior vertebral height restoration of the two groups were compared before and after operation.Results:All patients were successfully completed the operation and postoperative follow-up, there were no wound infection, breakage of internal fixation occurred, the paraspinal muscle approach group patients had the shorter operation time, the less bleeding volume during operation, the less postoperative drainage volume, the shorter post-operative bed time, and the VAS scores of 72H postoperative are significantly better than the conventional posterior median approach group( p<0.05) , There are no significant difference between the two groups of patients with vertebral Cobb angle correction rate be-fore and after operation and the anterior vertebral height restoration degree ( P>0 .05 ) .Conclusion:In the thoracolumbar burst fracture( Frankel E) , the paraspinal muscle approach is a feasible operation approach;Compare to the conventional posterior median approach, it has the same vertebral height restoration and Cobb angle correction, also has a shorter operation time, less bleeding, rapid recovery after operation, better satisfaction of patients , and it is worthy of promotion .关键词
