磁共振成像对乳腺钙化灶的诊断价值
Diagnostic values of magnetic resonance imaging in breast calcification

Abstract
Objective To identify the value of magnetic resonance imaging (MRI) in breast calcification which was non-calcified lesion in screen ultrasonography. Methods The study was done for 209 patients with 226 breast calcification by mammography (MG) classified as BI-RADS 4-5 and all negative by ultrasonography between January 2011 and March 2014. Forty-two malignant and 184 benign lesions were detected by both MRI and biopsy. Results The sensitivity of diag-nosis for malignant breast calcification were 69.0%and 85.7%by MG and MRI respectively ( P=0.068), and the specificity 64.7%and 88.0%(P<0.001). The sensitivity of diagnosis was higher by combination MRI with MG ( χ2=15.4 P<0.001) sig-nificantly than that by MG alone, and the negative predictive value increased from 90.2% to 100.0%. There was 78.6%(33/42) of malignant breast calcification as non-mass-like enhancement by MRI and 11.9% (5/42) as non-enhancement, while 78.3% (144/184) of benign calcification was non-enhancement (P<0.001). Malignant lesion was suggested when flow pattern of non-mass-like enhancement was present on MRI. The flow pattern was present in all malignant mass-like en-hancements. Tewenty-one (50%) from 42 malignant calcified lesions were carcinoma in situ (CIS). The other 21 lesions were infiltrating carcinoma. Conclusions This study showed that MRI has the higher sensitivity and specificity in differen-tiation of benign and malignant breast calcified lesions. The combination of MRI with MG could increase the diagnostic sensitivity compared with MG for the non-calcified lesion by ultrasound examination.关键词
