随机尿微量白蛋白与尿肌酐比值在妊娠高血压疾病早期肾损伤的临床应用
Clinical applications of random urinary albumin-creatinine ratio for early renal injury in patients with hypertensive disorders of pregnancy

Abstract
Objective:To evaluate the application of urinary albumin-creatinine ratio ( UACR) for early renal injury in patients with hypertensive disorders of pregnancy,and develop the corresponding reference value of UACR with mild or severe preeclampsia. Method:148 in-patients with hypertensive disorders of pregnancy were involved, who were hospitalized from Jan. 2013 to July 2014,which including 48 patients with gestational hypertension,55 with mild and 45 with severe preeclampsia, and 30 normal pregnant women in late pregnancy. Their UACR and urinary protein of 24h were determined. The results were analyzed by the receiver operating characteristic ( ROC ) curves in order to determine the corresponding diagnostic cut point of UACR with the urinary protein of 24h of 0. 3g and 0. 2g. Results:The average urinary protein of 24h in group gestational hypertension, mild and severe preeclampsia, and normal pregnant women in late pregnancy were (114. 91 ±52. 31) mg/24h,(779. 12 ±540. 85) mg/24h,(4159. 55±1719. 69) mg/24h and (80. 05 ±29. 68) mg/24h respectively. The values of UACR were in order (16. 91±7. 76)mg/ mmol,(75. 20±38. 10)mg/mmol,(486. 78±202. 73) mg/mmol and (1. 42±0. 45)mg/mmol respectively. There was no difference in urinary protein of 24h between group gestational hypertension and normal pregnant women in late pregnancy. However,the statistical differences existed between other groups when compared with each oth-er. And there was significant difference in UACR between each group. The UACR had a high linear correlation with urinary protein of 24 h in patients with gestational hypertension ( r=0. 929,P<0. 001). The values of UACR was 32. 85mg/mmol and 174. 5 mg/mmol correspon-ded with the urine protein of 24h of 0. 3g and 2. 0g respectively. Conclusion:There is high cor-relation between UACR and urine protein of 24h. UACR can be used for detection of early renal damage in those with hypertensive disorders of pregnancy. UACR,which can be regarded as an index for assessing the severity of pregnancy induced hypertension ( PIH ) , is expected to re-place the urine protein of 24h. The best diagnostic the cut point of UACR needs greater sample size for support.关键词
