Abstract
Objective:To investigate incidence and species distribution of pregnant women with thyroid dysfunction. These were to provide a theoretical basis for the necessity and possibility of the implementation of monitoring of iodine nutrition and thyroid function screening during pregnancy in Zibo in the future. Methods:1268 cases of pregnant women who had taken antenatal examination from Mar. 2013 to Feb. 2014 in Zibo maternal and child health hospital clinic were selected. The serum thyrotropin (TSH),free thyroxine (FT4) and thyroid peroxi-dase antibody ( TPOAb) were determined in early,middle and late pregnant women,by chemi-luminescence. Simultaneously urinary iodine in collected urine was measured by a cerium-con-tact method of arsenic acid digestion. Results:In 1268 cases of pregnant women surveyed,the median urinary iodine in first-trimester, second-trimester and third-trimester were 100. 3, 110. 5,105. 2. Urinary iodine level in first-trimester was the lowest (P<0. 05),and urinary io-dine level in second-trimester was higher than third-trimester ( P<0 . 05 ) . Reference value range of FT4 in first-trimester, second-trimester and third-trimester were recommended respectively with 12. 50~25. 10pmmol/L,12. 10~23. 10pmmol/L and 11. 20~20. 16pmmol/L. Reference value range of TSH in first-trimester,second-trimester and third-trimester were recommended re-spectively with 0. 15 ~3. 20mIU/L,0. 40 ~3. 90mIU/L,0. 50 ~4. 12mIU/L. Both FT4 and TSH in the three groups were in normal range. FT4 in both high-urine iodine group and low-u-rine iodine group was lower than that in normal urine iodine group ( P<0 . 05 ) . There was no significant difference in TSH among the three groups ( P>0 . 05 ) . In cases of pregnant women surveyed,368 cases of abnormal thyroid function,the incidence rate was 29. 02%. The inci-dence rate of abnormal thyroid function in first-trimester was higher than both second-trimester and third-trimester. Subclinical hypothyroidism occurred the most in abnormal thyroid function, followed by simple TPOAb positive. Conclusion:Pregnant women in Zibo are seriously lack of iodine nutrition and in need of urinary iodine levels detecting during Pregnancy. With the in-crease of gestational age,the FT4 values decreased,while the TSH value increased. Urinary io-dine abnormal can cause changes in FT4 early,but effect little on the TSH. Implementation of thyroid function screening is recommended for women with thyroid disease high-risk factors.关键词
