早期组织皮瓣覆盖和负压封闭引流术在下肢严重创伤合并大面积皮肤软组织缺损的应用效果
CLINICAL EFFECT OF EARLY TISSUE FLAP COVERAGE AND VACUUM SEALING DRAIN-AGE (VSD) ON LOWER LIMB TRAUMA WITH LARGE AREA SKIN AND SOFT TISSUE DEFECT

Abstract
Objective To explore the clinical effect of early tissue flap coverage and vacuum sealing drainage (VSD) on se-vere lower limb trauma with large area soft tissue defect .Methods 76 cases of severe trauma of lower extremities with large area soft tissue defect admitted to our hospital from May 2012 to Dec.2013 were randomized into two groups (42 cases in the treatment group and 34 cases in the control group).After thorough debridement, external fixation combined with limited internal fixation was applied to fracture sites stably in the treatment group .Then VSD was used to treat these wounds.After fresh granulation tissue grew , the tissue flap coverage and VSD were used to repair the exposed wounds at early stage .As for the control group after thorough debridement and fracture site fixation, conventional wound dressing change was used for replacing the VSD.After fresh granulation tissue grew, the tis-sue flap coverage was used to repair the exposed wounds.Results 42 cases in the treatment group after thorough debridement and VSD, the granulation tissues grew well.All the flaps survived at the primary stage without infections .The average hospital stay of the treatment group was 29.5 ±5.4d.34 cases from the control group developed wound infections with 5 cases as secondary wound heal-ing after flap transplant.The average hospital stay of the control group was 38.6 ±7.2d.All cases were followed up for 12 to 14 months.All the wounds and fractures healed with satisfactory appearance and no infection .The function of injured limbs recovered well.Conclusion Patients with severe lower limb trauma with large area soft tissue defect can be effectively repaired by tissue flap coverage and VSD at early stage, which will shorten the treatment time and promote early rehabilitation .关键词
