前外侧腹膜外入路一期病灶清除植骨内固定治疗腰骶段结核临床观察

Abstract
Objective To observe the effect of anterolateral extraperitoneal debridement and fusion with internal fixa-tion for treatment of lumbosacral tuberculosis. Methods A retrospective analysis of 21 cases of lumbosacral tuberculosis was performed, one stage extra peritoneal tuberculosis bone fusion and internal fixation were performed on all of these patients, postoperative follow-up of erythrocyte sedimentation rate (ESR), C reactive protein (CRP), X-ray, CT were performed to evaluate the conditions of TB. Results Postoperative follow-up observation was lasted for 9 -48 months, all the incision healed, all cases had no recurrence of tuberculosis. 6-12 months postoperative imaging revealed the fusion of intervertebral bone. At the last follow-up, 1 case’s spinal cord function from preoperative ASIA grade C recovered to grade D, 3 cases from grade D recovered to grade E, ESR from preoperative (38. 1 ± 12. 5) mm/h down to the (11. 3 ± 7. 1) mm/h, CRP from pre-operative (6. 2 ± 4. 2) mg/L down to the (1. 6 ± 1. 3) mg/L, VAS score from preoperative (4. 6 ± 1. 1) points down to the (1. 4 ± 1. 0), preoperative ODI score from (50. 2 ± 11. 9)% down to (13. 0 ± 6. 6)%,lumbosacral angle from preoperative (20. 0 ± 4. 8) degrees to the (29. 0 ± 3. 9) degrees. All the parameter was statistically significant different from before the operation ( P <0. 05). Conclusion Based on the conventional antituberculosis treatment,one stage anterolateral extraperitoneal and radical debridement bone graft fusion and internal fixation for the treatment of lumbosacral tuberculosis is safe and effective.关键词
