Abstract
Objective To discuss the effect of Tongxinluo capsule and irbesartan on early diabetic nephropathy ( DN) in patients with renal and endothelial function.Methods From April 2012 to June 2013, 100 cases of early DN were randomly divided into control group and observation group, 50 patients in the control group received conventional therapy and Irbesartan orally;observation group of 50 patients were received Tongxinluo capsules and Irbesartan.Before treatment and af-ter 12 weeks of treatment, fasting blood glucose, 2 h postprandial blood glucose, renal function, serum endothelin-1 (ET-1) and nitric oxide ( NO) changes were compared between the two groups.Results Before treatment, two groups’ serum creati-nine, urea nitrogen, creatinine clearance, 24h urinary total protein, urinary protein excretion, fasting glucose, 2 h postprandi-al blood glucose, NO, ET-1 showed no statistical significance difference ( t =0.571, t =0.561, t =0.593, t =0.576, t =0.553, t =0.526, t =0.568, t =0.551, t =0.587, P >0.05).Compared with before treatment, after 12 weeks of treat-ment, patients in the control group and the observation groups’ fasting plasma glucose, 2 h postprandial blood glucoses’ difference was not statistically significant ( t =0.594, t =0.602;t =0.613, t =0.594, P >0.05); serum creatinine, blood urea nitrogen, creatinine clearance, 24 h urinary total protein, urinary albumin excretion rate, ET 1 were decreased (t =1.948, t =1.958, t =1.809, t =1.847, t =1.895, t =1.825, t =1.869, t =1.880, t =1.876, t =1.846;t =1.814, P <0.05), NO were increased ( t =1.769, t =1.801, P <0.05); compared with the control group, observation group’s fasting glucose, 2 h postprandial blood sugar-free significant changed ( t =0.568, t =0.602, P >0.05), serum creatinine, urea nitrogen, creatinine clearance, 24h urinary total protein, urinary albumin excretion rate, ET 1 were signifi-cantly decreased ( t =2.478, t =2.509, t =2.447, t =2.785, t =2.658, t =1.998, P <0.05), NO were significantly increased ( t =2.036, P <0.05) .Conclusion It demonstrated that the Tongxinluo capsule and irbesartan could significantly reduce serum levels of ET-1 in patients with early DN and elevated the NO levels, significantly improved early renal function in patients with DN.关键词
