肥胖型多囊卵巢综合征患者超声特征及其与内分泌指标的相关性
Ultrasound characteristic and its correlation with endocrine profile in obese polycystic ovary syndrome patients

Abstract
Objective:To study the ultrasonic characteristic and its correlation with endocrine profile in obese poly-cystic ovary syndrome (PCOS) patients. Methods:One hundred and twenty-six patients with PCOS were recruited. Patients were categorized into PCOS with OB (OB-PCOS group, n=45) and without OB (NOB-PCOS group, n=81) groups according to body mass index (BMI). Ultrasonic parameters and hormonal profiles were obtained and compared, and their correlations were analyzed. Results: Compared with NOB-PCOS group, BMI, FN (follicle number) ,Vol (ovarian volume) and ratio of SA/TA (stromal area/ ovarian total area), uterine artery RI (resistance index), FT (free testosterone ), triglyceride (TG), in-sulin resistance (HOMA-IR), 2-h postprandial blood glucose(2hPG), fasting plasma insulin (FINS), 2-h postprandial plasma insulin (2hINS), fasting blood glucose (FPG) of OB-PCOS group were significantly higher (P<0.05), but RI of ovarian inter-stitial artery, sex hormone binding globulin (SHBG), high density lipoprotein (HDL)were significantly lower(P<0.05). Com-pared with OB-PCOS group, luteinizing hormone(LH), follicular stimulating hormone(FSH) of NOB-PCOS group were sig-nificantly higher (P<0.05). In OB-PCOS group, FN was correlated significantly with BMI, luteinizing hormone/follicular stimulating hormone (LH/FSH ) (r=0.31, 0.46, P<0.01). Vol was correlated significantly with BMI, FPG, glycosylated hemoglobin (Hb1AC ), HOMA-IR, and 2hPG (r=0.44, 0.50, 0.79, 0.44, 0.39, P<0.05);the ratio of SA/TA was correlated significantly with BMI (P<0.01). Uterine artery RI was correlated significantly with androstendione(AD)(r=0.52, P<0.05). RI of ovarian interstitial artery was correlated significantly with BMI (r=-0.28, P<0.05). In NOB-PCOS group, FN was corre-lated significantly with LH/FSH (r=0.38, P<0.05); Vol was correlated significantly with FT (r=0.46, P<0.05). Conclusions:There are certain ultrasonic characteristics in obese polycystic ovary syndrome patients, and correlations exist between ul-trasound and endocrine parameters. Categorization of polycystic ovary syndrome patients by BMI combined with ultrasonic characteristics, endocrine and metabolic indices has reference value for the evaluation of disease, risk assessment of metabolic syndrome and prevention of long-term complication.关键词
