丹参川芎嗪治疗慢性阻塞性肺疾病急性加重期并肺源性心脏病的临床观察

Abstract
Objective:To observe efficacy and mechanism of salvia miltiorrhiza and tetramethylpyrazine in the treatment of acute exacerbation of chronic obstructive pulmonary disease(COPD) accompanied by pulmonary heart disease .Methods:A total of 90 patients with acute exacerbation of COPD accompanied by pulmonary heart disease were randomly selected and divided into two groups ,the salvia miltiorrhiza and tetramethylpyrazine group and the control group ,with 45 cases in each .Both groups were given symptomatic treatment ,and the salvia miltiorrhiza and tetramethylpyrazine group received 5 mL salvia miltiorrhiza and tetramethylpyrazine injection(dissolved in 250 mL 5% glucose solution) by intravenous infusion once a day in addition .Both the two groups have been treated for 15 days .The blood gas analysis indexes(PaO2 ,SaO2 ) ,the hemodynamic indexes(PASP ,PADP ,MPAP) ,and the vasoactive factor indexes(ET‐1 ,NO) of the patents in two groups were detected before and after the treatment .Results:There was no statistically significant difference between the two groups in each index before the treatment .Each index of patients in both two groups improved after the treatment ,compared with that before the treatment(P< 0 .05、0 .01) , of which the patients in salvia miltiorrhiza and tetramethylpyrazine group improved more significantly .Conclusions:Salvia miltiorrhiza and tetramethylpyrazine showed good efficacy on acute exacerbation of COPD accompanied by pulmonary heart disease .It could improve the oxygen supply for lung tissue ,reduce the generation of ET‐1 , increase the release of NO ,reduce the PASP and improve the cardiopulmonary function .关键词
