Abstract
Objective To explore the clinical features of cognitive dysfunction in patients with multiple system atrophy (MSA)and the related factors. Methods 34 MSA out - patients or inpatients in neurology department,General Hospital of PLA from December 2012 to November 2013 were chosen in the study,including 21 patients with cerebellar - predominant (MSA - C),13 patients with parkinsonism - predominant(MSA - P),another 18 healthy old people were chosen as the control group,who had similar gender,age,educational background and living atmosphere to the MSA patients. Three groups were carried out whole cognitive assessment,executive function assessment and anal sphincter electromyogram examination. Results MSA - P group were older but had longer years of education than the other two groups( P ﹤ 0. 05). MSA - C group had lower MoCA scores than the control group(P ﹤0. 05). MSA - C group and MSA - P group took longer time in Stroop test card A,B, D than the control group and MSA - C group took longer time in card C(P ﹤0. 05). MSA - P group had lower scores in clock drawing test than the control group(P ﹤0. 05). The conversing number of MSA - C group and MSA - P group in digital - symbol/ graphics - symbol conversion quiz was fewer than that of the control group(P ﹤0. 05). MSA - P group took longer hours in trail - making tests than the control group(P ﹤0. 05). In semantic fluency score,MSA - C group and MSA - P group were lower than the control group,MAS - C group was lower than MSA - P group(P ﹤0. 05). According to anal sphincter electromyogram examination results,19 cases were in electromyography normal group and 15 in abnormal group;the abnormal group took longer hours in Stroop test card D and in trail - making tests and shorter hours in clock drawing test than the normal group(P ﹤0. 05). Multiple linear regression analysis showed that semantic fluency was negatively correlated with schooling years( r = - 0. 443,P ﹤0. 05);trail - making test was positively correlated with ages(r =0. 462,P ﹤0. 01);the abnormal rate of electromyography was negatively correlated with clock drawing test score(r = -0. 411,P ﹤0. 05) and positively correlated with trail - making test time,the time - spending in Stroop test card D(r =0. 473,0. 455,P ﹤0. 05). Conclusion Cognitive impairment occurs in patients of multiple system atrophy,especially the executive dysfunction;cognitive impairment is more severe in patients with multiple system atrophy of abnormal anal sphincter electromyogram examination.关键词
