摘要
Abstract
4CH8 community health management model is a new method which was proposed by several experts. Through 4 steps of health management,directing at 4 groups of key population for health management,this model is to construct 4 groups of people under health management and the corresponding management places and add 8 self-testing health modules for the residents. It brings a new sense to community health management. On the basis of summarizing domestic and foreign health management theories and practice,This article focuses on the constructing background of the 4CH8 model and analyzes its appli-cation advantages and prospect in the community health management by combining with the concrete practice of the model by Shanghai Pudong New Area Sanlin community.关键词
