Abstract
Objective To investigate the main factors influencing hospitalization expenses of the patients having me-chanical ventilation( MV)in intensive care unit( ICU),to provide a theoretic evidence for controlling the unduly rapid increa-ses of medical expenses and diminishing patients' burden. Methods A path analysis was performed in 166 MV patients admitted to a provincial hospital from January 2008 to December 2012. Their general information and clinical examination results were ob-served and recorded. Forward stepwise method was used to analyze regressively to fit path analysis model,to screen the direct and indirect effects on hospital costs and calculate their sizes. Results The effects of factors influencing MV patients' hospital costs were,in descending order,hospital stay,ICU stay,intubation,MV time,Ventilator-associated pneumonia( VAP), infection,enteral nutrition,the year of hospital admission,treatment outcome,albumin supplement. VAP treatment outcome, intubation had not only direct effect but also indirect effect on hospital costs. VAP,ICU stay,MV were positively correlated with hospital stay,costs(P<0. 05),and treatment outcome,hospital infection,enteral nutrition,albumin supplement negatively with hospital stay,costs(P<0. 05). Conclusion Hospital stay,ICU stay,MV time,intubation,VAP,hospital infection are major influencing factors of hospital costs. The government,medical institutions,medical insurance institutions should coop-erate to take comprehensive measures to effectively control the excessive growth of hospital costs of ICU MV patients.关键词
