静脉注射联合气道吸入抗生素治疗呼吸机相关性肺炎效果的 Meta 分析

Abstract
Objective To evaluate the efficacy of the antibiotic intravenous combined with inhalation of antibiotics for the ventilator - associated pneumonia(VAP). Methods Literatures about antibiotic intravenous combined with inhalation of antibiotics for VAP which were published from January 1990 to February 2014,were retrieved from the PubMed,EMBase, Cochrane Library,CBM,CNKI and Wanfang Database. The literatures which met the inclusive criteria were analyzed by the Review Manager 5. 2 software. Results A total of 8 papers(590 patients)were included,among whom there were 307 patients in the intervention group(antibiotic intravenous combined with inhalation of antibiotics therapy)and 283 patients( antibiotic intravenous therapy)in the control group. According to Meta - analysis results,there was significant difference in the VAP cure rate between two groups〔 RR = 1. 15,95% CI(1. 03,1. 29),P = 0. 01〕,but there was no significant difference in the mortality〔RR = 1. 93,95% CI(0. 77,1. 62),P = 0. 57〕,the incidence of adverse reaction〔RR = 1. 13,95% CI(0. 73, 1. 76),P = 0. 57〕and the incidence of drug - resistant bacteria〔 RR = 0. 41,95% CI(0. 07,2. 42),P = 0. 33〕between two groups. Conclusion The antibiotic intravenous combined with inhalation therapy can improve the cure rate of VAP,but has no obvious effect on the mortality,the incidence of adverse reaction and the incidence of drug - resistant bacteria.关键词
