Abstract
Objective To investigate the relationship between glycosylated hemoglobin( HbA1c )in pregnant women with gestational diabetes metullis ( GDM ) and the risk of neonatal hypoglycemia ( NH ). Methods A total of 604 GDM pregnant women were entered who were admitted to Anqing Navy Hospital between May,2010 and May,2014,and received regular antenatal examination. They were divided into three groups according to HbA1c level before delivery:group G1(n=375, HbA1c <5. 5%),group G2(n=108,HbA1c 5. 5% -6. 5%)and group G3(n=121,HbA1c >6. 5%). The blood glucose of the neonates was detected 30 min,2 h,6 h,12 h after birth respectively. Results In the 604 cases of newborns,there were 120 cases with NH(19. 9%)and the incidence was 15. 2%(57 cases) in group G1,21. 3%(23 cases)in group G2, and 33. 1%(40 cases)in group G3. The blood glucose level of normal newborns was lower in G2 and G3 than that in G1 30 min after birth,lower in G3 than that in G1 and G2 2 h after birth and lower in G3 than that in G1 12 h after birth. The blood glucose level of newborns with NH was lower in G2 and G3 than that in G1 30 min,2 h,6 h after birth and was lower in G3 than that in G1 12 h after birth and the 30 min of blood glucose levels after birth in the three groups were all lower than 2. 2 mmol/L. The level of prenatal HbA1c was negatively correlated to the level of blood glucose of NH newborns 30 min after birth(r= -0. 401,P<0. 05). Conclusion The higher level of HbA1c in pregnant women with GDM,the higher incidence of NH newborns, especially when the level of HbA1c is higher than 6. 5%. The 30 min after birth is the key early time point for diagnosis of NH and the blood glucose of NH newborns should be corrected as early as possible.关键词
