Abstract
Objective To investigate the diagnostic value of different diagnostic methods on patients with heart failure with preserved ejection fraction. Methods We enrolled 146 patients suspected of heart failure with preserved ejection fraction who were treated in Zhenhai Longsai Hospital from March 2012 to November 2013. All subjects were diagnosed with mitral flow Doppler spectrum,tissue Doppler imaging(TDI)and brain natriuretic peptide( BNP)combined with N - terminal pro brain natriuretic peptide(NT - proBNP). Comprehensive analysis on relevant clinical data and the diagnostic results by the three methods was conducted by two experienced doctors of the hospital,and a golden standard was obtained by their discussion. The diagnostic value of the three different diagnostic methods on heart failure with preserved ejection fraction was compared and analyzed. Results Among the 146 subjects suspected of heart failure with preserved ejection fraction,the number of subjects who were definitely diagnosed was 105 by the golden standard,68 by mitral flow Doppler spectrum,83 by TDI and 82 by BNP combined NT - proBNP. The sensitivity of the diagnosis by mitral flow Doppler spectrum,TDI and BNP combined NT - proBNP was 61. 9% ,66. 7% and 68. 6% respectively; the specificity of the three methods was 92. 7% ,68. 3% and 73. 8%respectively;the Kappa value of the three methods was 0. 428,0. 301 and 0. 377 respectively. The cost of TDI was lower than that of mitral flow Doppler spectrum and BNP combined NT - proBNP. TDI and BNP combined NT - proBNP had better operability than mitral flow Doppler spectrum( P < 0. 05). Conclusion Indicators of sensitivity,cost,operability,and practicability showed that TDI and BNP combined NT - proBNP are better diagnostic methods in the noninvasive examination of heart failure with preserved left ventricular ejection fraction. Diagnostic methods should be selected according to specific situations in clinical practice.关键词
