甲状腺功能正常的2型糖尿病患者校正QT间期延长的影响因素研究
Influencing Factors for Increased QTc Interval Duration in Patients With Type 2 Diabetes Mellitus and Normal Thyroid Function

Abstract
Objective To investigate the risk factors for increased QTc interval duration in patients with type 2 diabetes mellitus and normal thyroid function. Methods By using systematic sampling method,246 patients with type 2 diabetes mellitus who were admitted into Department of Endocrinology,Yiyang Central Hospital from January to December in 2013 were enrolled as case group and 103 healthy people who received physical examination in the hospital during the same period were enrolled as control group. General data of the subjects were collected,including gender,age,course of disease and BMI. Glycosylated hemoglobin(HbA1c),fasting C-peptide(FCP)and postprandial C-peptide(PCP),thyroid function,blood lipid and homocysteine( Hcy)were tested. QTc interval duration was an value of QT duration corrected by Balett formula,QTc=QT/ RR,for heart rate influence. QTc>0. 44 s was defined as increased QTc interval duration,by which case group and control group were further divided into normal QTc subgroup and increased QTc subgroup. Results Rate of increased QTc was 22. 8%(56/246) for case group and 17. 5%(18/103) for control group,with significant difference between the two groups (χ2 =4. 900,P <0. 05). In case group,normal QTc subgroup and increased QTc subgroup had significant difference in gender,age,course of disease,BMI,HbA1c,FT3,FT4,TSH,TC,LDL-C,Lp - α,Hcy(P <0. 05). In control group,normal QTc subgroup and increased QTc subgroup were significantly different in age,TC and Lp-α(P<0. 05). The Logistic regression analysis showed that gender〔OR =1. 012,95%CI(1. 002,1. 022)〕,course of disease〔OR =1. 173, 95%CI(1. 066,1. 280)〕,BMI〔OR = 1. 122,95% CI(1. 012,1. 232)〕,HbA1c〔OR = 1. 097,95% CI(1. 007, 1. 187)〕,FT3〔OR=0. 950,95%CI(0. 901,0. 999)〕,FT4〔OR =1. 136,95%CI(1. 042,1. 229)〕,TSH〔OR =1. 136,95%CI(1. 043,1. 229)〕,LDL-C〔OR =1. 019,95%CI(1. 004,1. 034)〕and Hcy〔OR =1. 542,95%CI (1. 254,1. 830)〕were influencing factors for increased QTc interval duration in patients with type 2 diabetes mellitus( P <0. 05). Conclusion Patients with type 2 diabetes mellitus have high rate of increased QTc interval. Blood glucose,thyroid function,blood lipid are influencing factors.关键词
