颏下皮瓣在咽瘘修复中的应用

摘要
Abstract
To investigate the clinical effect and operation methods of using submental flap to repair of pharyngocutaneous fistula after total laryngectomy.Methods:7 cases with pharyngocutaneous fistula after total laryngectomy were selected from June 2006 to July 2010,5 cases of laryngeal carcinoma after operation,2 cases of hypopharyngeal carcinoma after operation.Pathological examination were all showed squamous cell carcinoma.Internal fistula with 1.5~3.5 cm were saw in the operation.Sutured the donor skin directly after releasing free.Results:All the flaps survived;donor site healing,no obvious scar. Extracted the stomach tube and oral feeding after 14~21 days,nobody had difficulty swallowing.The follow-up time ranged from 23 to 78 months(averaged 47 months),Patients satisfacted with the repair effect of incision operation,no one acuured hypopharyngeal stricture and swallowing difficulties.Conclusion:Submental flap has the advantages of nearly pharyngeal fistula, abundant blood supply,hidden incision,higher survival rate,so it is one of the better method for repair of pharyngocutaneous fistula after total laryngectomy.关键词
