临床药师在慢性疾病治疗管理中的作用
The role of clinical pharmacists in the treatment of chronic disease management
摘要
Abstract
With the increase of chronic disease patients,the treatment of chronic diseases is getting more and more attention.For the treatment of chronic disease is a long-term or even lifelong treatment process,that need professional doctors,pharmacists, nurses to provide comprehensive,continuous,active treatment management for patients with chronic diseases,in order to achieve the purpose of promote the rehabilitation,slow the disease process,and reduce complications.Clinical pharmacist is a group of clinical pharmacy new strength.In guidance or in clinical pharmacotherapy,we used medication follow-up,medication education and medication consultation and other pharmaceutical service mode to involve the chronic diseases treatment.We received the obvious treatment effect,increased medication compliance,the re hospitalization rate was reduced,and improved the quality of life. It played a decisive role in the chronic disease management,so it is indispensable.关键词
