Abstract
Objective To summarize the experiences in the diagnosis and treatment of nonfunctioning pancreatic neuroendocrine tumor. Methods The clinical data of 52 patients underwent resection for NF-PNET between 2006 and 2013 in Peking Union Medical College Hospital were analyzed retrospectively. The clinical characteristics, diagnosis and treatment were studied. Results A variety of digestive tract symptoms or symptoms caused by local tumor compression were the common chief complaint and no endocrine disorder was detected. Diagnostic accuracy of enhanced CT scan and ultrasonography was 90.39%and 79.61%respectively. A total of 52 patients underwent surgical resection including 44 patients performed radical excision, 7 patients performed primary tumor resection plus hepatic metastasis resection, 1 patient performed biopsy of primary tumor and hepatic metastasis. According to the grading system,there were 16 patients with G1 (30.77%),21 patients with G2 (40.39%) and 15 patients with G3 (28.84%). Statistical analysis demonstrated that tumor diameter is positively correlated with the lymph node metastasis (P=0.001),vascular invasion (P=0.047) and liver metastasis (P=0.047). Conclusion NF-PNET is often with nonspecific symptoms. Enhanced CT scan is an important diagnostic method. Operation is the primary therapy for NF-PNET. When the tumor size is less than 2 cm,patients are less likely with lymph node metastasis,vascular invasion and liver metastasis. So the tumor is probably can be removed and lymph node resection is not necessary.关键词
