Abstract
Objective To enhance the pharmacist self-management consciousness,to reduce the number of outpatient pharmacy dispens-ing errors and to ensure the medication safety of patients. Methods The SHEL mode,i. e. ,software(S):professional quality and ability of medical staff;hardware(H):hardware conditions;environment(E):clinical environment;litigant(L):parties and the others,was adopted to analyze the error situation and the error occurrence causes in the outpatient pharmacy from June 2012 to June 2013. The relevant im-provement measures were put forward. Results and Conclusion The main factors causing error are the staff themselves business defi-ciency,the imperfect relevant system,backward hardware equipment and chaotic drug management. The application of the SHEL model is helpful to improve the quality of outpatient pharmacy safety management,eliminate the potential safety hazard of drug dispensing and re-duce the occurrence rare of the dispute events between pharmacists and patients in order to serve the patients better.关键词
