摘要
Abstract
OBJECTIVE:To explore the role of clinic pharmacist in warfarin anticoagulation treatment . METHODS: Clinic pharmacist participated in consultation and provided pharmaceutical care for one patient with cerebral embolism following aortic mechanical valve replacement.RESULTS:Clinical pharmacists analyzed the causes of cerebral embolism, adjusted warfarin anticoagulation therapy and provided pharmaceutical care for the patient. CONCLUSIONS:Clinical pharmacist's involvement in warfarin anticoagulant treatment may lead to increase of success rate yet decease of the risk of anticoagulant therapy.关键词
