41次临床药师会诊分析
Analysis of Forty-one Consultations Attended by Clinical Pharmacist

Abstract
Objective: To study the characteristics of pharmaceutical consultation participated by clinical pharmacist and to provide a reference for the improvement of the quality of pharmaceutical consultation . Methods:Retrospective analysis was conducted of 41 times of clinical pharmacist consultations for 24 patients in our hospital during August to December of 2013 . Results: There were progress notes in the medical records of pharmacist consultations . Among 41 times of consultation , 36 times ( 87 . 8%) was related to the application of antibiotics , 3 times ( 7 . 3%) related to alternative medicines , and also related to the scheme of diagnosis and treatment and medical safety analysis , etc . About 87 . 8% of the comments from clinical pharmacist consultations was all accepted , and 7 . 32% was partially accepted , as the results , 15 patients ( 62 . 5%) were cured and 8 patients ( 30%) improved . Conclusion: The pharmacist had opportunities to attend repeated consultations and the whole process of patient ’ s medication . There is some clinical demand for the pharmacist consultation and a high compliance was showed for the suggestions of pharmacist consultations . It was suggested by clinical pharmacist that the safety and effectiveness of clinical treatment should be enhanced and clinical pharmacist should keep learning and practice to improve consultation quality .关键词
