Abstract
Disease Management can improve the quality of medical treatment and reduce the unnecessary medical expenditure . Licensed pharmacist can play an important role in disease management , especially in chronic disease management . This article introduced the role of licensed pharmacist in disease management and the strategies for licensed pharmacist in the chronic disease management were proposed .关键词
